Job Title/Position: Quality Management Nurse Reports To: Chief Nursing Officer/Administer QUALITY MANAGEMENT NURSE JOB DESCRIPTION SUMMARY The Quality Management Nurse is responsible for ensuring the highest quality patient care through correct documentation and staff development expertise as an educator, consultant, facilitator and change agent. Will also, provide appropriate and continual training, oversight and mentoring to assist in the development and performance improvement of clinical personnel. Furthermore, this position participates and contributes as an important member of a collaborative team environment to promote the organization/agency as an effective member of the health care delivery system and employer choice. QUALITY MANAGEMENT NURSE ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Audits and monitors patient chart compliance by reviewing patient's clinical diagnosis, medications, procedures, clinical course and makes recommendations to clinical supervisory and administrative team members within regulatory and accreditation standards. Be available to clinical staff as a resource to field questions regarding establishing immediate and long-term therapeutic goals, in setting priorities and in developing a plan of care and continually instructs and guides clinical staff as appropriate. Assists in the identification of skills for annual competence and develops educational programs and in-service training for clinical staff and incorporates principles of adult learning with a focus on improvement of skills and retention of knowledge particular to orientation, documentation, staff development and/or safe practice. Ensures that all orientation, competency evaluation, and in-service education are appropriately documented per agency policy. Maintains current knowledge related to Medicare Conditions of Participation and the Accreditation Commission for Health Care (ACHC) standards to ensure that the organization is compliant with state, federal and accreditation guidelines. Responds to changes in clinical practice and regulation while incorporating them into the programs to support those changes. Attends care conferences with organization personnel to facilitate coordination of care and educate on correct documentation and performance improvement. Serves as a role model to promote excellence and continual learning. QUALITY MANAGEMENT NURSE POSITION QUALIFICATIONS Demonstrates organization, coaching, and expertise in mentoring/teaching skills with excellent understanding of the principles of adult learning. Experience in an educational role is preferred. Ability to work as a member of a highly collaborative team. Ability to perform in a high-paced, growth-oriented environment. Minimum of five (5) years of clinical experience in home health care. Knowledge of ICD-10 Coding and OASIS D Guidance. Demonstrates skills of high detail orientation, coordination, prioritization, is self-directed and able to work with minimal supervision. Has an excellent understanding of performance improvement, quality assurance, and utilization management. Must be a registered nurse with current State licensure to practice professional nursing. A graduate of National League for Nursing accredited School of Nursing. BSN degree preferred. Demonstrates excellent observation, verbal and written communication skills. Complies with accepted professional standards, practice and regulating bodies. Complies with, and maintains current CPR certification and other requirements pursuant to company policy. Basic computer skills, personal technology tools, and awareness of the current use of technology in a related field. Advanced computer skills, the technology of the specialty preferred. Must have proficient skills with Microsoft Office. Knowledge of value-based purchasing, Knowledge of Home Health 5-star ratings Knowledge of HHCAPS.
