Job Description Job Description SUMMARY Assists clients and
families in the behavioral health system to enhance their recovery
and promote wellness. Assists Recovery Coaches in providing
rehabilitation and support services. DUTIES Promotes a
recovery-based approach to care that emphasizes hope, respect,
self-direction, and empowerment. Assists clinical team in
implementing services plans. As applicable, works directly with
members on a peer driven, self-developed recovery plan to be shared
with their Behavioral Health Medical Practitioner (BHMP) that
includes goals of the member / family, community resources,
family/natural supports, and crisis prevention plan. Works with
clients and/or their family and natural supports to navigate and
use community resources in an effective manner. Provides support
and rehabilitation services in the community or client’s home, in
office, or telephonically as appropriate. Consults with, makes
referrals to, and follows up with other community agencies to
provide needed services. Completes documentation for behavioral
health record in accordance with agency and funding source
requirements. Relates own recovery process or family experience in
a positive and supportive manner through coaching, role modeling
and mentoring to assist clients and/or their families in developing
skills to achieve their goals (if certified to provide peer or
family support services). Communicates with clinical team regarding
client’s level of independent functioning and current levels of
support provided. Provides crisis support as needed and notifies
Supervisor/team. Assists client in completion of legal consents and
benefit coaching. Transports clients to appointments that will
assist them with their recovery plan. Assists with client
appointments that promote wellness and recovery. Assists in
assessment of hospitalized/crisis clients for purpose of discharge
planning. Performs welfare checks in the client’s home to assess
for health and safety risks and reports clinical information to the
Recovery Coach/Supervisor Attends required training. Attends
required meetings. Coordinate all care and services required to
enhance and facilitate the optimal physical and behavioral health
wellness of persons served within a trauma-informed care framework.
This includes: 1) facilitating communication between medical and
behavioral health practitioners/systems 2) assisting with
scheduling of and compliance with preventative and specialty
physical health interventions 3) integrating treatment
interventions to address both the physical and behavioral health
needs of the person/family concurrently 4) assist persons/families
served with education and resources outlining the interplay of
physical and behavioral health issues on overall wellness and
quality of life. An individual may qualify for the position if the

In order to provide peer support services, individuals must have
personal recovery experience and be certified as a recovery support
specialist through one of the recognized training agencies.
Individuals must be 21 years of age or older. KNOWLEDGE, SKILLS,
and ABILITIES Ability to work with multi-disciplinary adult
recovery teams. Knowledge of the Pima County behavioral health
system. Ability to engage clients in treatment plan. Good
communication skills. Ability to work independently. Uses good
judgment as it relates to peer/family support. Knowledge of
confidentiality, ethics, and boundary issues related to peer/family
support. Other Potential Requirements Reliable transportation,
Valid AZ driver’s license, automobile insurance, and good driving
record. Bilingual (Spanish/English) preferred. Proof of physical
exam and annual proof of TB skin Test. Fingerprint Clearance - If
